TITLE: Social Worker
REQUIREMENTS: Master’s Degree in Gerontology, Social Work, or related and 6 months of experience in social work services.
SPECIAL REQUIREMENTS: Familiar with assessment tools such as the Geriatric Depression Scale (GDS) and Mini-Mental State Examination (MMSE), proficient in Microsoft Word and Excel, with strong communication and organizational skills.
JOB DUTIES: Assess clients’ physical, emotional, and social needs. Develop care plans, provide counseling, coordinate services, and ensure access to benefits. Conduct outreach and maintain compliant records.
